ABSTRACT

Objectives: The impact of the COVID-19 pandemic on mental health is not yet well-studied. This study's objective is to describe demographic characteristics of the population diagnosed with depression or anxiety, and to compare PHQ9 scores before and after the pandemic. Method(s): A retrospective cohort study was performed using Komodo Health's healthcare claims and EMR data, which included Patient Health Questionnaire-9 (PHQ9) survey responses. The study's baseline and follow-up periods were set as one year before and after 03/01/2020. Patients selected were >=18 years of age, had a MDD, GAD, or other psychiatric diagnosis in both periods, and had taken at least one PHQ9 survey in both periods, resulting in 10,433 patients. Demographic characteristics were described across age, gender, and race/ethnicity, and a subgroup analysis was performed on PHQ9 scores and depression categories using averages (mean, SD) and odds ratios. Result(s): Demographic analysis showed depression severity correlated with patients who were younger, female, and Black or Hispanic. Younger patients (<30) were more likely than older (>=30) to be in the moderately severe category or worse (PHQ9 score >=15) in both time periods (ORs 1.72 and 1.62, p<0.001). This was also true for female as compared to male (ORs 1.45 and 1.49, p<0.001), and Black or Hispanic as compared to White (ORs 1.87 and 1.47, p<0.001). However, mean PHQ9 scores tended to decrease in the follow-up period. The overall mean decreased slightly from 6.28 (SD 6.05) to 5.68 (SD 5.82), which was consistent in nearly all age, gender, and race/ethnicity subcategories. Conclusion(s): While the improvements in average PHQ9 scores were counterintuitive, given the harmful impacts of the pandemic, existing correlations between demographics and depression severity remained. One possible explanation is that this cohort definition selected for patients who received more consistent mental healthcare. ABSTRACT

INTRODUCTION: Anti-Asian racism was a feature of the social response to the Covid-19 pandemic, and its impact on the well-being of Asian communities warrants closer examination. The current study aimed to gauge whether the sense of belonging mitigated the adverse effects of racism on life satisfaction for self-identified Asian New Zealanders.METHODS: This analysis included 1341 responses to a cross-sectional online survey conducted in 2021. Descriptive analyses outline how components of a sense of belonging were distributed among participants and those who experienced racism during the Covid-19 pandemic. We used linear regression to examine the role of a sense of belonging as a potential pathway variable in the association between experiencing racism and life satisfaction.FINDINGS: In this survey, four out of 10 participants reported experiencing racism in the first 18 months of the pandemic. Participants' life satisfaction decreased slightly since January 2020 (p<0.001). Experiencing racism was associated with decreased life satisfaction. All the components of sense of belonging reduced the magnitude of this negative association between racism experience and life satisfaction, in particular, expressing one's own ethnic identity and belonging in Aotearoa.CONCLUSIONS: Given that anti-Asian racism is currently a feature of life and a significant stressor during the pandemic, this study provides empirical evidence of the protective role of a sense of belonging against anti-Asian racism. This study focused on Asian members in Aotearoa New Zealand, but its practical implications have the potential to support other minoritised ethnic communities who also experience racism during the pandemic and beyond.

ABSTRACT

Objectives: Long-COVID, the persistence of various symptoms after COVID-19 infection, is still not fully understood. This study evaluated the incidence and chronicity of post-COVID-19 conditions using administrative claims, which provide a large, generalizable sample, to provide insight into the scope of long-COVID in the United States. Method(s): Patients newly diagnosed with COVID-19 from 4/1/2020-3/31/2021 were identified in the MerativeTM MarketScan Commercial and Medicare databases. The first COVID-19 diagnosis served as the index date and patients were continuously eligible for 12-months pre- and post-index. Incident conditions were defined as a new diagnosis (no pre-period diagnoses) for one of 17 conditions of interest in the first 60-days of the post-period. Among patients with an incident condition, chronicity of the condition was assessed over the remaining post-period (long-term conditions). Result(s): The sample included 503,742 patients;mean+/-SD age was 39.5+/-16.5 and 46% were male. The most common incident conditions were respiratory symptoms (24.1%), fatigue (7.3%), muscle pain (6.0%), and headache (5.9%). Among patients with each of these conditions, long-term persistence was observed in 21.9% for respiratory symptoms, 36.8% for muscle pain, 18.3% for fatigue, and 16.0% for headache. Fewer than 5% patients evidenced incident anxiety, mood disorders, myocarditis, sleep disorders, or pulmonary embolism;however, among these patients, over 40% had long-term persistence of the condition. Among patients with long-term conditions, sleep disorders (248+/-98 days), mood disorders (239+/-96 days), anxiety (236+/-95 days), respiratory symptoms (233+/-92 days), and asthma (230+/-93 days) had the longest average durations of persistence, evidenced by continued claims over the post-period. Conclusion(s): With the continued presence of COVID-19 an understanding of the risk of long-term symptoms is needed to help manage patients both during and following infection. These findings provide some initial insight into the incidence and tenure of various conditions that are affecting patients diagnosed with COVID-19.Copyright © 2023

ABSTRACT

BackgroundPatients with autoimmune inflammatory rheumatic diseases are at higher risk for coronavirus disease (COVID)-19 hospitalization and worse clinical outcomes compared with the general population. However, data on the association between COVID-19 outcomes and gout, or gout-related medications are still lacking.ObjectivesWe aimed to compare COVID-19 related clinical outcomes in gout vs. non-gout patients.MethodsWe conducted a retrospective cohort study using the electronic health record-based databases of Seoul National University hospital (SNUH) from January 2021 to April 2022 mapped to a common data model. Patients with gout and without gout were matched using a large-scale propensity score (PS) algorithm. The clinical outcomes of interest were COVID-19 infection, severe COVID-19 outcomes defined as the use of mechanical ventilation, tracheostomy or extracorporeal membrane oxygenation, and death within 30 days of COVID-19 diagnosis. The hazard ratio (HR) for gout vs. non-gout patients derived by Cox proportional hazard models were estimated utilizing a 1:5 PS-matched cohort.Results2,683 patients with gout and 417,035 patients without gout were identified among the patients who visited SNUH. After 1:5 PS matching, 1,363 gout patients and 4,030 non-gout patients remained for the analysis. The risk of COVID-19 infection was not significantly different between patients with gout and those without gout (HR 1.07 [95% CI 0.59-1.84]). Within the first month after the COVID-19 diagnosis, there was also no significant difference in the risk of hospitalization (HR 0.57 [95% CI 0.03-3.90], severe COVID-19 outcomes (HR 2.90 [95% CI 0.54-13.71]), or death (HR 1.35 [95% CI 0.06-16.24]).ConclusionPatients with gout did not have an increased risk of COVID-19 infection or worse clinical outcomes. ABSTRACT

We investigated the impact of human activity during COVID-19 on the tropospheric nitrogen dioxide vertical column density (NO2 TropVCD) at three urban sites (Gwangju and Busan in Korea and Yokosuka in Japan) and one remote site (Cape Hedo in Japan) from Multi-Axis Differential Optical Absorption Spectroscopy (MAX-DOAS) and Pandora. Compared to the monthly mean NO2 TropVCD from 2015 to 2018 and in 2019, the values were lower in 2020 due to social distancing in Korea and Japan. High negative relative changes were observed from May to September (-30% to -18%) at the three urban sites;Cape Hedo, a remote site, did not show a significant difference in relative changes between previous years and 2020, suggesting that only anthropogenic emission sources decreased dramatically. In the case of Yokosuka, the 15-day moving average of the NO2 TropVCD exhibited a good relationship with transportation (R = 0.48) and industry (R = 0.54) mobility data. In contrast, the NO2 TropVCD at the Korean sites showed a moderate to low correlation with the industrial sector and insignificant correlations with transportation. The differences in correlations might be caused by the different social distancing policies in Korea (voluntary) and Japan (mandatory). By applying generalized boosted models to exclude meteorological and seasonal effects associated with NO2 TropVCD variations, we revealed that the decreasing trend from 2019 to 2020 was much steeper than that from 2015 to 2020 (a factor of two), and a significant change was identified in January 2020, when the first cases of COVID-19 were observed in both Korea and Japan. This result confirmed that the reduction in NO2 can be largely explained by the NOx emission reduction resulting from social distancing for COVID-19 rather than annual meteorological differences;however, in December 2020, NO2 recovered suddenly to its previous level due to an increase in human activities.

ABSTRACT

Background: Although our understanding of immunopathology in the risk and severity of COVID-19 disease is evolving, a detail of immune response in long-term consequences of COVID-19 infection remains unclear. Recently, few studies have detailed the immune and cytokine profiles associated with PASC. However, dysregulation of immune system driving pulmonary PASC is still largely unknown. Method(s): To characterize the immunological features of PPASC, we performed droplet-based scRNA-sequencing using 10X genomics to study the transcriptomic profiles of peripheral blood mononuclear cells (PBMCs) from participants naive to SARS-CoV-2 (NP, n=2) and infected with SARS-CoV-2 with chronic pulmonary symptoms (PPASC, n=2). Result(s): Analysis of more than 34,000 PBMCs by integrating our dataset with previously reported control datasets generated cell distribution and identified 11 immune cell types based on canonical gene expression. The proportion of myeloid-lineage cells (CD14+monocyte, CD16+monocyte, and dendritic cells) and platelets were increased in PPASC compared with those of NP. Specifically, PPASC displayed up-regulation of VEGFA and transcription factors, such as ATF2, ELK, and SMAD in myeloid-lineage cells. Also, TGF-beta and WNT signaling pathways were up-regulated in these cell population. Cell-cell interaction analysis identified that myeloid-lineage cells in PPASC participated in regulation of fibrosis and immune response, such as VEGFA (increased) and MIF (decreased) interactions. Conclusion(s): Together, this study provides high-resolution insights into immune landscape in PPASC. Our results emphasize differences in myeloid lineage-mediated fibrosis and immunity between PPASC and NP, suggesting they could act as potential pathological drivers of PPASC. (Figure Presented).

ABSTRACT

Introduction: With the increasing detection of incidental pulmonary nodules (IPNs), there is a clinical need for a dedicated IPN service to ensure that growing PNs are managed in a timely manner. Pre COVID-19, our centre ran a virtual nodule service, delivered on an ad-hoc basis by the lung cancer physicians. We hypothesised that efficiency of the service would improve with a dedicated nodule team. We were awarded a pump priming grant by the Thames Valley Cancer Alliance to implement a nodule navigator run service. We report the initial outcomes of this project here. Objective(s): To evaluate the PN navigator service. Method(s): Retrospective data pre-service development was collected from patients presenting to the PN service between April and June 2022. The service was established in October 2022 and data from October and November 2022 collected. Student t-test was used to compare means. [Table presented] Results: 107 patients were included pre-service and 92 patients in the post-service development cohorts. Data for time to CT report and patient contact are summarised in Table 1. There was no reduction in mean time from CT scan date to CT report (Table 1;31vs 27;p=0.143) but a reduction was seen between CT report and patient contact (Table 1, 45 vs 20;p<0.001). Conclusion(s): This small cohort study shows an improvement in the time between CT scan and patient contact following the introduction of a dedicated PN service. This may reduce delays in the diagnosis of early-stage lung cancer. Whilst there was no significant difference between the CT scan date and CT report, these data highlight an area in the pathway that can improve. ABSTRACT

Artificial Intelligence (AI) is a transformative force in communication and messaging strategy, with potential to disrupt traditional approaches. Large language models (LLMs), a form of AI, are capable of generating high-quality, humanlike text. We investigate the persuasive quality of AI-generated messages to understand how AI could impact public health messaging. Specifically, through a series of studies designed to characterize and evaluate generative AI in developing public health messages, we analyze COVID-19 pro-vaccination messages generated by GPT-3, a state-of-the-art instantiation of a large language model. Study 1 is a systematic evaluation of GPT-3's ability to generate pro-vaccination messages. Study 2 then observed peoples' perceptions of curated GPT-3-generated messages compared to human-authored messages released by the CDC (Centers for Disease Control and Prevention), finding that GPT-3 messages were perceived as more effective, stronger arguments, and evoked more positive attitudes than CDC messages. Finally, Study 3 assessed the role of source labels on perceived quality, finding that while participants preferred AI-generated messages, they expressed dispreference for messages that were labeled as AI-generated. The results suggest that, with human supervision, AI can be used to create effective public health messages, but that individuals prefer their public health messages to come from human institutions rather than AI sources. We propose best practices for assessing generative outputs of large language models in future social science research and ways health professionals can use AI systems to augment public health messaging. © 2023 ACM.

ABSTRACT

This study aimed to understand how the COVID-19 pandemic impacted the stress levels and life satisfaction of individuals with disabilities and what role social support plays in mitigating these impacts. Data from individuals with disabilities (n = 600) were extracted from a larger international cross-sectional survey study. Regression analyses were conducted to examine how personal and environmental factors contributed to stress levels and life satisfaction changes during the pandemic. Mediation analyses were used to test whether social support mediated the association between stress level and life satisfaction. The negative impact of the pandemic, having a psychiatric disability, being a female, and being younger significantly predicted stress levels. Moreover, the negative impact of the pandemic and stress levels significantly predicted life satisfaction. The relationship between stress levels and life satisfaction was mediated by social support from significant others and friends. There is an urgent need to provide training and education for healthcare professionals, families, and caregivers on the basic aspects of mental health care, developing time-limited and culturally-sensitive mental health interventions, and strategies for expanding the social support system for the population. Broad dissemination of effective therapeutic strategies is crucial in mitigating the negative psychosocial impact of the pandemic.

ABSTRACT

Disposable surgical face masks are designed to block the airborne transmission of infectious viruses between patients and healthcare personnel;however, there are limitations to a surgical mask's ability to prevent transmissions because the mask does not provide an airtight seal around the face. This study focuses on the design and development of a custom mask frame, using individuals' three-dimensional (3D) facial scans, to improve the seal of the existing surgical masks. Three participants' facial scans with a wide range of facial dimensions were collected using a 3D hand-held scanner. During design, the contact area between the custom frame and the wearer's skin was determined based on the individual's 3D facial shape. To evaluate the effectiveness of the airtight seal, a qualitative fit (smell/taste) test kit and thermal imaging camera were utilized. When the participants wore the surgical masks without the customized mask frames, the participants smelled the saccharine scent from the test kit. However, while wearing the surgical masks fitted with the custom mask frame that held the mask in place, none of the participants could smell the scent. In addition, thermal images showed ambient temperature changes in the surgical mask suggesting air leakage around the boundary of the mask while the subject was inhaling and exhaling. With the mask frame, thermal images confirmed the effectiveness of the custom frame to block the inflow/outflow of air potentially carrying the virus. The results indicate that the custom-built mask frames are effective in providing a proper and much improved seal compared to a surgical mask without the frame. The developed custom mask frame will be particularly beneficial for frontline medical staff members who treat patients with infectious viruses transmitted through aerosols as part of their PPE.

ABSTRACT

The COVID-19 crisis provided an opportunity for information professionals to rethink the role of information in individuals' decision making such as vaccine uptake. Unlike previous studies, which often considered information as a single factor among others, this study examined the impact of the quantity and trustworthiness of information on people's adoption of information for vaccination decisions based on the information adoption model. We analyzed COVID-19 Preventive Behavior Survey data collected by the Massachusetts Institute of Technology from Facebook users (N = 82,213) in 15 countries between October 2020 and March 2021. The results of logistic regression analyses indicate that reasonable quantity and trustworthiness of information were positively related to COVID-19 vaccination intent. But excessive and less than the desired amount of information was more likely to have negative impacts on vaccination intent. The degrees of trust in the mediums and in the sources were associated with the level of vaccine acceptance. But the effects of trustworthiness accorded to information sources showed variations across sources and mediums. Implications for information professionals and suggestions for policies are discussed. © 2023 Association for Information Science and Technology.

ABSTRACT

Purpose: This study aimed to identify the factors that affect post-traumatic growth among frontline nurses during a COVID-19 pandemic. Methods: This study included 187 nurses working in nationally designated infectious disease hospitals as participants. Data were collected from January 11 to March 2, 2021 using structured questionnaires. Independent t-tests, one-way ANOVA, Pearson correlation coefficients, and hierarchical multiple regressions were performed to examine influencing factors of post-traumatic growth. Results: Posst-traumatic growth was positively correlated with traumatic event experience (r=.26, p<.001), post-traumatic stress (r=.32, p<.001), supervisor support (r=.39, p<.001), and colleague support (r=.36, p<.001). Factors affecting post-traumatic growth were emotional support of supervisors (β=.76, p<.001) and evaluative support of colleagues (β=.46, p<.018). Overall, approximately 40.0% of the variability in post-traumatic growth was explained by these two variables. Conclusion: To increase emotional support of supervisors and evaluative support of colleagues, the most influential factors for post-traumatic growth of frontline nurses who fought against COVID-19, a positive culture must be established. This includes horizontal communication, a safe working environment, and securing of appropriate nurse to patient ratio. © 2022 Korean Academy of Nursing Administration

ABSTRACT

The rapid expansion of plastic manufacturing industries in last several decades has brought serious concerns over the environmental impacts of plastic wastes. Recent outbreak of Covid-19 drastically increased production, use, and disposal of plastic products. Current management strategies for wasted plastics still rely on landfill and incineration that continue to exacerbate plastic pollution and carbon emissions. Many countries have put forward multifaceted administrative efforts to reduce plastic wastes, but the annual global generation of plastic wastes is still increasing. In techno-society, researchers have been exploring more effective plastic wastes treatment technologies to alleviate environmental impacts of plastic wastes. Such efforts entailed several technical options that can potentially contribute to establishing a circular economy for plastics. Thermochemical process is a prominent example of such techniques. This review presents an overview of the issue of plastic pollution, covering topics including global plastic production, environmental impacts, and toxicity. In addition, the global administrative efforts aimed at reducing plastic pollution are discussed, as well as detection and treatment strategies to establish a circular economy in plastic management. © 2023 Elsevier B.V.

ABSTRACT

This paper examines the effects of working from home on mental health, with particular attention to the role of home environments. Using unique real-time survey data from South Korea collected during the COVID-19 pandemic, we find that working from home negatively affects the mental health of workers, with greater effects on women and those who are primarily responsible for housework while also maintaining market work. Surprisingly, workers who live with children in the household do not suffer from the negative effects of working from home. Our findings suggest that family-work interaction may be an important factor in the optimal design of working from home. © 2023 Institute of East and West Studies, Yonsei University, Seoul.

ABSTRACT

Although lung disease is the primary clinical outcome in COVID-19 patients, how SARS-CoV-2 induces lung pathology remains elusive. Here we describe a high-throughput platform to generate self-organizing and commensurate human lung buds derived from hESCs cultured on micropatterned substrates. Lung buds resemble human fetal lungs and display proximodistal patterning of alveolar and airway tissue directed by KGF. These lung buds are susceptible to infection by SARS-CoV-2 and endemic coronaviruses and can be used to track cell type-specific cytopathic effects in hundreds of lung buds in parallel. Transcriptomic comparisons of infected lung buds and postmortem tissue of COVID-19 patients identified an induction of BMP signaling pathway. BMP activity renders lung cells more susceptible to SARS-CoV-2 infection and its pharmacological inhibition impairs infection by this virus. These data highlight the rapid and scalable access to disease-relevant tissue using lung buds that recapitulate key features of human lung morphogenesis and viral infection biology.

ABSTRACT

We suggest that one source of understanding entrepreneurial resilience and regional culture is through the denizens of Silicon Valley, and how they are handling crisis situations. We surveyed 119 entrepreneurs, investors, and large company executives based in Silicon Valley during the first COVID-19 lockdown period between April and May 2020. Our data show that the entrepreneurs perceived the situation as severe but were cautiously optimistic. They embraced creative destruction and took it as a way to change the status quo for the better. They predicted the new normal in the industry by acknowledging the critical role of science and technology. The ecosystem seems to enhance entrepreneurial resilience in the region through the social structures and entrepreneurial cultural values. © 2021 The Author(s). On the other hand, the winter heating system currently used in railway vehicles is very vulnerable to spreading of respiratory infectious diseases such as COVID-19 by convection of hot air, so the need for a new heating technology is being raised. In this study, a composite material laminate panel manufacturing technology capable of generating heat was developed using a paper-type carbon fiber sheet. To this end, the resin infusion method was used to evaluate the effect of resin flow due to insertion of carbon fiber sheet during panel molding, and quantitative evaluation of the heating performance of the manufactured specimen was performed. As a result of the experiment, the applicability of a heat-generating composite material laminate panel to a new rail vehicle heating system was confirmed. © 2022 The Korean Society for Railway. ABSTRACT

The demand for face masks is increasing exponentially due to the coronavirus pandemic and the particulate matter (PM) in the atmosphere. As a result, an enormous number of disposable mask filters have been produced and discarded, contributing to plastic waste. Underprivileged people who cannot afford to purchase commercial face masks have started making fabric masks with waste clothing;however, this material does not effectively filter viruses or PM. Therefore, in this study, a chitosan coating was applied to clothing fabrics to increase their effectiveness as face masks. The improvement in the PM removal efficiency owing to the chitosan polymer was observed for stocking, innerwear, and bamboo materials, but not for cotton. Furthermore, chitosan prepared in the form of a nanowhisker (CsW) achieved a PM 2.5 removal efficiency of 96% in a five-layer cotton fabric. In addition, a commercial biodegradable poly(lactic acid) filter was coated with CsW, which increased the PM 2.5 removal efficiency from 67% to 83%. Additionally, microbial growth was significantly suppressed in the chitosan-coated fabrics, and the degree to which it was suppressed depended on the coating concentration.
